Depends on the context or company. Game design might cover concept art, story, writing, programming, etc.
Game art and animation is a bit more straight forward, but can mean 3D or 2D. It might mean character concepts and design, environment concepts, actual 3D models, and animations (such as a fighting stance or an attacking animation on a game)
Chances are that in general, game design will likely cover more on the process of making the design (from the concept to the programming, likely more on programming and the process games go through)
Game art and animation likely flat out means the character concepts and designs onto bringing them into life via 3D models and animations (or pixel ones, depending on what type of game it is in general)
